Keep health details for your consultation

Please use the request form only for your name, treatment interest and general availability. Do not include medical history, photographs or other sensitive details.

External services

The online booking calendar is provided by Cal.com, which processes the booking information you enter. Opening directions takes you to Google Maps. These services apply their own privacy terms.
